Having fears of relapse

I have not been on in about a month due to school starting (I'm a teacher) and just the hecticness of the start of a school year and being busy but I am on today cuz I'm a nervous wreck.  Aidan is in LTM for Tcell ALL and has been fine.  He was off meds for a week or so due to low counts and now they are down to 50% of 6mp and 33% of methotrexate (normal steroid dose for the 7 days after vinc) and on bactrim.  When off the meds his counts went up (higher then we've seen since dx but not out of normal range, still scared me).  Went back on the above dose and counts almost 2 wks ago were back to where is expected for LTM (not too low but not too high). 
Well last night I noticed some very small but swollen cervical lymph nodes in Aidan's neck.  I noticed it when he turned his neck and then I could feel them on the one side and then again on the other.  They are very tiny and a little tender.  But I am scared as the swollen lymph nodes were the only presenting symptom up to 2 days before Aidan's dx last year (ironically it was around this same time last year that those were discovered).  Though the very swollen ones last year were submadibular whereas these are definitely behind the ear and likely the cervical lymph nodes.  In addition to this he's had a very mild rash on his face (maybe sun exposure or???0 no where else and this rash seems to come and go.  He has some darker circles under his eyes today but the wind is blowing.  I did not find any other enlarged nodes, he's totally his normal energetic self.  No petichea.  I did see a little redness in his throat but he didn't really say it is bothering him.  I am scared to death that the lymph nodes could be enlarged due to relapse.  He goes on Tues to his onc for his LP and Vinc...but I'm nervous mainly cuz this was his only presenting symptom.  I called the oncall onc who thinks it could just be due his previously higher counts or a little virus/cold that caused it but I'm scared to death and could barely sleep last night.   I asked how a relapse could present and she mainly said as blasts in the blood but I tried to tell her his cbc at dx was normal even when his nodes were very large and it wasn't until weeks later that his counts were incredibly high!

 

Anyone else experience this w/their child?  Any of these meds cause this symp?  She also said the nodes may have been this tiny for awhile I just have not noticed them.  Any advice for the appt on Tues or things to ask?

Its so hard....when my daughter was first dx her symptoms were:

constant illness for 1 1/2 months

tierd

pale

 

she didn't have any bruising, no nose bleeds

 

When she relapsed her main symptom was brusing. I first noticed an eraser size bluish red "dot" on the inner part of her elbow, petechia. That was the only little spot. I was a nervous wreck with this one little spot. I sent her off to school telling myself she was fine only to have the school nurse call me to tell me "did you know she has this wierd bruise on her arm"  plus she got hit in the back of her calve with a lunch cart and instantly a huge grapefruit sized bruised formed. She was dx with a relapse at her next appt which was 2 days from getting that wierd bruise. Those were her only symptoms. When she relapsed the second time it was more like her initial dx, illness, tierd, pale.

 

I think relapse is so different in every child. Does your child seem to be fighting a little bug? Maybe the redness in his throat? I know my daughter would get strep and we wouldn't even have known it unless they did the strep test. I guess because counts are so low they sometimes don't have enough of them to react to viruses and show no symptoms. Maybe have  a strep test done while you are there to rule it out.

 

I hope all is well at his next appt. So many ups and downs with this darn disease....

Ahhh, worry. My constant companion. I honestly don't know what my brain would do all day if I did not worry. :-)

 

As for your current worry, both our cancer kids have had slightly swollen glands in their neck throughout treatment and LTM. We have about 1 year left of LTM. Usually these same glands--for them they are usually in their neck right up high under their jaw, and it happens when they are fighting a cold or whatever and they can stick around for a few days after they are better. Does Aiden have a runny nose or cough?

 

When Seth was dx, he had the whole host of symptoms -- bruising, nose bleeds, unexplained fever, aches in his joints, etc. Micah, on the other hand, was just a bit pale with large glands in his neck and in the all around his collar bone, etc. He also had a swollen spleen, which I have no idea what that looks like/feels like, but that is a classic symptom.

 

This past summer, Micah went off meds for 2 weeks when his counts were low. His wbc went really high after that (like 10,000 or 10.1), but his white cells were like 85 or 90% neutrophils. He never really came down with anything, but the high neuts were what was making his wbc high, not blasts or even lymphocytes. The rest of his counts were normal and he went right back to a "regular" wbc a couple weeks later. So even a high wbc doesn't mean relapse. I have always heard that a replapse will show up in the counts -- I always took that to mean high wbc, probably with blasts, but not always, and low plts and red cells. Wouldn't it be great if someone invented a tester like diabetics use that we could do a cbc from home! :-)

 

You mentioned a rash...is it red and almost pimply around Aiden's mouth/nose? That is what my guys have and I remember they had it last winter, too. It went away during the summer. I am thinking dry cold air and lack of suntan is what is going on, but I was planning to ask about this at our next visit. Anyone else have this? I have heard that steroids can cause acne, even in the youngest kids.

 

Aspiedel, hang in there. I worry myself sick just about once a week. I take a tylenol pm before bed so that I can get some rest. And I visit this site. Right now, both Micah and Seth seem pale to me. Then I started comparing their skin to our healthy kids skin and they look the same. Just no sun shine for so long. We live in MN and are in a long spell of rain and clouds and even snow. I looked around church this morning and the only people with a healthy glow are the men who work outdoors all the time. :-)

My boys are not on steroids right now either. They start again on Wednesday, so we are at the same point in the month...hmm....I will definitely ask our onc about this. Maybe it is something 6mp or mtx can cause, since that is what they take all the time. I should go back and find those info sheets on all the drugs they get and see what the 6mp and mtx sheet says.

I can't comment on the other symptoms, but Peter gets acne type spots round his mouth and on his chin from the 6mp. He also gets them all over his neck. The higher the dose of 6mp he is on, the more spots he gets.

I will be thinking of you guys and really hoping that Aidan is not relapsing.

Aspiedel,

 

How did your clinic visit go on Tuesday?  Well, I hope. We had ours on Wednesday and we talked with the onc about the rash/dry skin stuff going on with the boys' faces. He didn't really know what could be causing it. He suggested waiting until we were done with the steroid pulse and then using hydrocortinzone cream for a couple days just to see if that helped. Since the Eucerin was not doing much. But, we are 4 days done with dex and the rash is about gone. How is Aiden?
