I’ve added units and comments. The blood test results come with units and it is important to include them because not every lab uses the same units. It is easy to start comparing apples to oranges without explicitly stating units.
WBC 923 per mm3 … very low
HGB 9.6 per deciliter … anemia
HCT 30.0 … low, consistent with anemia found above
MCV 141.9 femtoliter … very high, anemia classified as macrocytic anemia
PLT 51 per nanoliter (also Giga/L) … very low
Your relative is definitely having problems with his blood system. I am not surprised that they are checking him for CLL even though the usual B-cell CLL often shows up with excess lymphocytes. There are T-cell versions of CLL that sometimes exhibit low lymph counts. His Mean Cell Volume number struck me the most. He has MCV = 141.9 fL which means his red blood cells are more than 50% larger than normal. Perhaps, the spleen is removing the enlarged red blood cells at a rapid rate. This can cause splenomegaly. It can also lead to anemia because the bone marrow cannot keep up with red blood cell replacement. Something is definitely amiss. (And it does not have to be B-CLL. The FISH test on flow cytometry will be very helpful.)
I also have macrocytic anemia with my hgb ranging between 8 to 10 g/dL and my MCV around 120 fL. I can alert you to one annoying question that I’ve had to field over the years: do you abuse ethanol? It seems alcoholics are notorious for developing macrocytic anemia. I take proactive stance and tell them to forget that angle of attack.
It is early going for your relative. He may need a bone marrow biopsy for a definitive diagnosis. I wish him the best of luck.
